Enzymatic estimation of infarct size. Thrombolysis induced its demise: will it now rekindle its renaissance?

چکیده
I nfarct size was central to the major thrust of cardiac research in the 1970s and as such occupied center stage as the culprit to be conquered by interventions designed to cardioprotect and limit myocardial damage (infarct size). Myocardial infarction, with its consequent necrosis, was the leading cause of death and came under intense attack with infarct size posing as the surrogate villain. Investigators were unified in their attack on infarct size, namely, that its course must be charted, the extent of damage quantified, and ultimately therapy designed for its elimination. Studies demonstrated that the evolution of myocardial infarction is a dynamic process and that its rate of evolution, as well as the ultimate extent of damage, could be favorably modified.1,2 Quantitative histologic estimates of infarct size were regarded as the only gold standard but obviously were not possible in patients surviving acute myocardial infarction. Attempts to quantitate

Effects of pretreatment with non hypotensive dose of ramiprilat and losartan on myocardial ischemia-reperfusion induced arrhythmias and infarct size in rats
Introduction: Inhibition of renin angiotensin system represents an important approach in the management of cardiovascular diseases. The aim of this study was to explore the effects of pretreatment with non-hypotensive dose of angiotensin converting enzyme (ACE) inhibitor, ramiprilat and angiotensin type 1 (AT1) receptor blocker, losartan on myocardial infarct size and arrhythmias in a rat mo...
متن کامل'Expected infarct size without thrombolysis', a concept that predicts immediate and long-term benefit from thrombolysis for evolving myocardial infarction.
BACKGROUND Thrombolytic therapy should only be used when expected benefits outweigh the risks. In order to obtain a precise estimation of prognosis, with and without thrombolytic therapy, we postulated that mortality reduction by thrombolytic therapy is a function of the area of myocardium at risk for necrosis. We developed a model to estimate the myocardial area at risk for necrosis from clini...

Pump failure, ranging from ventricular dysfunction to acute cardiogenic shock, is now the leading cause of cardiac death. Efforts at temporary mechanical or pharmacologic support of the heart have been largely unsuccessful so that attention is now directed toward prevention of ventricular failure and limitation of myocardial infarct size or even outright prevention of infarction itself. In part...

See article by de Groot et al. [9] (pages 315 –324) in this limitation of enzymatic infarct size [7]. Earlier, we demonissue. strated that thrombolytic therapy had no influence on the relation between infarct size estimated with aHBDH and Although the use of biochemical markers of acute left ventricular performance, indicating that coronary remyocardial infarction started in 1954 [1], their val...
متن کاملEffects of early intracoronary streptokinase on infarct size estimated from cumulative enzyme release and on enzyme release rate: a randomized trial of 533 patients with acute myocardial infarction.
The effects of early intracoronary streptokinase (SK) on enzymatic infarct size and rate of enzyme release were studied in a randomized multicenter trial. A total of 533 patients with acute myocardial infarction (AMI) were allocated to either the SK treatment group (n = 269) or the conventional (control) treatment group (n = 264). Enzymatic infarct size was represented by the cumulative quantit...
